This activity has been planned and implemented in accordance with the Essential Areas and Policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint sponsorship of the University of Virginia School of Medicine and Public Health and Discovery Health Channel. The University of Virginia School of Medicine and Public Health is accredited by the ACCME to provide continuing medical education for physicians.

The University of Virginia School of Medicine and Public Health designates this educational activity for a maximum of 1 AMA PRA Category 1 Credit™. Physicians should only claim credit commensurate with the extent of their participation in the activity.

Program Overview
Chronic anticoagulation is indicated in the management of a number of medical conditions. However, warfarin has a narrow therapeutic window and requires regular monitoring to maintain efficacy and lower the risk of complications. Traditional office-based anticoagulation management has a number of practical and medical disadvantages.

The use of point of care testing has been shown to increase time in therapeutic range and decrease complications. Discovery Health has assembled a group of experts to discuss the benefits of point-of-care testing, the accuracy and precision of point-of-care instruments, and the appropriate patients for self-monitoring and self-management.

Learning Objectives
At the conclusion of this program, participants should be able to:

  • Describe the various models of anticoagulation management and their associated risks.
  • Discuss the benefits of patient self-monitoring of oral anticoagulation care.
  • Understand the accuracy and precision of point-of-care PT instruments compared to standard laboratory testing.
  • Describe the criteria for patient selection for patient self-monitoring of anticoagulation care as well as point-of-care PT testing.
  • Implement a program of patient self-monitoring and/or point-of-care testing of oral anticoagulation care.

Internists, Family Practitioners, Cardiologists, Cardiothoracic Surgeons, Neurologists, Nurse Practitioners and Physician Assistants
